Newly registered patients: Permanent
To register as a patient one must come to the surgery and complete a registration form. This must include your name, date of birth, address with postcode, ethnicity, first language and NHS number if known. You must also give us your previous address and previous GP details. Once the patient has completed these details, we will then enter their details onto the computer and arrange for medical records to be obtained from the previous GP.
Patients are informed of their named accountable GP at the time of registration.
In order to improve care for elderly patients aged over 75yrs a named GP is allocated, who is responsible for the care of this group of patients in case of an emergency.
We do not exclude patients from the practice on grounds of age, sex, colour, race, nationality, ethnic or national origin or disability, sexual orientation, religion or religious or philosophical belief or medical condition.
New arrivals to UK should bring their passport and will be registered provided they are entitled to NHS treatment within our practice boundary.
Please note we are currently only registering new patients who fit the following criteria:
1) Patients are living within our Practice Boundary.
2) Patients outside are Practice boundary will be registered if a) they are children born to parents who are already registered with us. b) They are the spouse of a patient who is already registered with us.
If a visitor to your home needs to see a doctor you may make arrangements for them to be seen at the practice on a temporary basis (up to a maximum of three months) provided they are entitled to NHS treatment.
Overseas visitors / private patients
Visitors from some countries are not entitled to NHS treatment but we can treat them privately which means they will need to pay for the treatment. For more information on this please contact the reception staff in person.
